signing this statement, I acknowledge that I have been informed
about the conditions associated with participation in the Dual
Degree Program that includes the MBA degree at the University of
Texas at Arlington and the Master of International Management at
The American Graduate School of International Management. Please
initial next to each provision listed below after you have read
it and have received answers to any questions that your may
have.

Provisions #1: I understand that I must apply
to each school separately and must clearly indicate on each
application an interest in this dual degree program. I
understand that I must be accepted into both programs in order
to qualify for the dual degree program.

Provision # 2: If I am enrolled in either
program I understand I may petition for admission to the dual
degree program through the appropriate channel at that
institution. If approved, I understand that I must also gain
acceptance to the other program.

Provision #3: I understand that I must meet all
requirements for both degrees and file the documents and fees
required by each institution in order to receive that degree.

Provision #4: I understand that I must plan
both programs of work and receive approval from the appropriate
advisors.
________
Provision # 5: I understand that I may use placement
services at both institutions during the final semester of
study, regardless of my enrollment during that semester.

Provision # 6: I understand that the transfer
of coursework under this dual degree program (12 hours to each
school) shall not be granted to individuals who are not approved
dual degree candidates.

Provision # 7: I understand that only those
courses on the approved joint course list may count toward the
transfer credits mentioned above.
